After reading this thread, and doing a bit of research, I will say that this comes down to disguise vs Spot skill.
Therefore, Masks & Hoods will give +10 to disguise.
Please note, spot rolls get a -1 vs Disguise for every 10 feet away from someone you are.
This is my ruling, mostly because this discussion comes up over and over again and a hard rule is what it sounds like we need.
By the take 10 rule of standing NPCs and such, you'll never be identified without their having higher spot skill.
To me, spot does not mean you "See better than others", it means to me you "Notice things better than others", considering what you roll against. So, noticing who someone is should come down to spot skill.
Now you can argue, "I Have mask and hood, HOW? You can't see through it!" but I agree with the argument that there's so many possibly distinguishing marks, mannerisms and more that have little to do with your hair and mouth being covered. Hell, you could have some distinguishing features about your eyes.
For example, I bet some of you can guess who this is:
