I have always preferred the subtle insanities. I had a wonderfully fun elf wizard that had survived being an orc captive by finding a "new friend" that he talked to a lot (but only in Draconic cuz that is all his friend spoke). Most thought he was full of it until they got to know him and he let them actually see his friend...his rabbit familiar named Harvey that thought he was a dragon :P. Yev and Asura both remember Ash (shortened from Ash'Ta'Azza because that was too hard to scream when running for your life from a bunch of orcs).
One good thing to keep in mind while playing a "touched" toon: madness does not usually vanish when an event starts or combat happens or especially when it is way out of place. Too many people play characters with insanities (and play them well) but slip back to "normal" at times when the situation makes such quirks detrimental. Insanities are really fun when they actually get in the way of success.(at least to me).
Personally, I find any over the top personality to be quite annoying after a while. Insane or just totally obnoxious...no difference to me. I get bored and just walk off after a few minutes. I've seen people play insane, overly inquisitive, and seriously concieted characters quite well. Then others take it a bit over the top and become experts on EVERYTHING or so insane you can't hold a conversation for more than a minute.
