It's opposed attack rolls on each persons current highest attack bonus. So, having power attack or expertise on puts you at a disadvantage when you disarm or are disarmed, because your attack bonus at that time is much lower. You get a bonus for a two-handed weapon, and a penalty for a light weapon. And if we wanted disarm to be accurate, it shouldn't have a cool down at all, it should be able to be spammed and simply replace your attacks as you que it up, similar to how smites work. Disarm is an attack action, not a standard.
